LSI currently seeks a Full time Care Coordinator Support Worker within our Family Safety, Risk, and Permanency program to serve clients in Howard, Winneshiek, and Allamakee counties.
The Care Coordinator Support Worker will provide the Care Coordinator direct assistance to achieve identified family goals for safety, permanency and well-being as specified within the Service Plan. The Care Coordinators Support Worker supports the Care Coordinator to ensure families receive the required level of services, supports and resources to ensure timely and safe reunification. Care Coordinator Support Worker is primarily responsible for facilitating and monitoring supervised family interactions as well as providing transportation. Meet with assigned families in person via traveling by car to their homes or other community settings and contact by telephone, which may occur during normal business hours, after hours, or on weekends.
Required: High school diploma with a minimum of one year of full time experience in child welfare services or human services; or an Associates degree with a minimum of six months of full time experience in child welfare services or human services; or Bachelor's degree.
Preferred: Bachelor's degree in a human service related field with a minimum of six months of full time experience in child welfare services or human services.
This position requires a valid driver's license, auto insurance, a good driving record and the ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.
