Social Workers and related fields, join our Integrated Health team and be part of healthcare transformation. LSI is seeking a full-time and/or part-time Behavioral Health Care Coordinator in our Pediatric Integrated Health program. Looking for someone who is dedicated to working with a transformative health program to empower members served. Bilingual (English-Spanish) is a plus, but not required.
Position is based in the Waterloo office, but will perform field work in Cerro Gordo, Floyd, Chickasaw, Franklin, Butler, Bremer, Blackhawk, Buchanan, Grundy, Tama, Benton and surrounding counties.
The focus of this position is a team approach to coordinate the child's physical and mental health care, as well as support community needs in order to enhance health outcomes. Additionally, the Behavioral Health Coordinator will make certain all health care providers have the information they need to provide quality care to enrolled clients, conduct client assessments, develop care coordination plans, and provide education and resources.
A Bachelors of Social Work or human behavioral science field from an accredited institution together with the equivalent of one year of full-time professional social worker experience acquired after the degree in a public or private social services agency is required. Applicants must be comfortable working in a continually changing environment, be a team player, a self starter and a creative thinker. Case management experience is preferred. Knowledge of child welfare system is preferred and knowledge of community resources is essential, as well as competency in computer basics and documentation skills. Acceptable human behavior science degrees are: psychology, sociology, human services/development, child development.
